Beauty and Reiki waiver: consent and release form
A consent and release form for beauty and wellness services records what the client told you about their skin and health, confirms they understand that treatments can cause reactions, and asks them to release you from ordinary-negligence claims to the extent the law allows. This page builds one for salons, estheticians, lash and nail techs, Reiki practitioners and similar services.

What goes in a consent and release for beauty services
Most disputes in appearance and wellness services start with a reaction, an expectation or a misunderstanding about what was done. A signed form helps by capturing the client's statements about allergies, skin conditions, medications and pregnancy before the service, and by recording that they were told reactions can happen.
The template makes no health claims. It states that the services are not medical care and are not a substitute for care from a licensed health professional. Licensing rules for beauty and wellness work are set by each state, and you must follow them; this tool does not tell you what they are.

What the template covers
The document includes an acknowledgment of risks, voluntary participation, a health statement, release of ordinary-negligence claims to the fullest extent permitted by law (gross negligence and intentional or reckless misconduct are not released), covenant not to sue, indemnification, optional emergency authorization, optional photo consent for before and after pictures, optional emergency contact block, optional parent or guardian consent, severability, entire agreement, governing law with your state, and signature lines.
For repeat clients, pair it with the client intake form from this site, which collects contact details and health notes.
Tips for practitioners
Ask clients to update their health information at every visit and keep a record. Patch tests and product details are part of professional practice and are outside the scope of this tool. Check what your license, your insurer and your landlord require, and have a licensed attorney in your state review the final wording.
Build it in four steps
- Choose beauty, Reiki and wellness. Preselected on this page.
- Review product and touch risks. Chemicals, heat and wellness risks are preselected.
- Add details and options. Photo consent and emergency contact are optional.
- Preview and edit. Adjust wording for your services.

Questions and answers
Do I need a waiver for Reiki or esthetics services?
No rule makes every practitioner use one, but many do to record client statements and reactions. Whether it helps depends on your state and the facts, so ask a licensed attorney in your state. Check your license rules too.
Does this form make medical claims?
No. It states that the services are not medical care or a substitute for care from a health professional, and it makes no claims about results or benefits.
Can I collect photo consent for before and after pictures?
Yes. Turn on photo and media consent in step 5. It adds checkbox-style lines so the client can say yes or no.
Should clients re-sign every visit?
The document is written to apply to every visit until replaced. Many practitioners ask clients to confirm their health information at each appointment.
